To provide financial assistance to young adults as they move forward with their lives after cancer. Qualifications: Applicants must be U.S. resident; must be between 21-39 years of age at time of application; not tax dependent in most recent year’s tax filing; must not receive a SAMFund grant in the previous grant cycle; Medical Selection criteria: Selection will be based on the submitted application materials. How to apply: Applicants must submit online application form; should include: a medical history verification form that is completed and signed by a licensed practitioner that can verify applicants cancer diagnosis (e.g., doctor, nurse, social worker, patient navigator); electronic signatures are accepted; recent Tax Forms, or an affidavit stating applicants have not filed taxes in either, or both, of the past two years; electronic signatures are accepted.
