To support Wisconsin students with sickle cell disease who plan to pursue undergraduate studies. Qualifications: Applicant must be a resident of Wisconsin who has been diagnosed by a physician with Sickle Cell Disease; must be graduating high school seniors, previous graduates, or current undergraduate students who are enrolled or plan to enroll in full-time undergraduate study at an accredited two-year or four-year college, university, or vocational-technical school for the upcoming academic year; must have a minimum 2.0 GPA. Selection criteria: Selection is based on academics, work experience, participation and leadership in school and community activities, unusual personal or family circumstances, statement of educational and career goals, and online recommendation. How to apply: Applicant must complete the online application and submit current transcripts along with a Diagnosis Verification Form.
