To provide financial assistance significant financial aid, and families are also responsible for contributing to the tuition. Qualifications: Applicants must be an 8th grader in the Current academic year; possess academic ability, leadership potential and financial need; reside in the Chicago-area or Lake County. Selection criteria: Selection will be based on committee criteria. How to apply: Applicants must be submit online application form; acceptable financial documents include: individual tax return (must include form 1040 and w2); applicant must be listed as a dependent; social security statement; supplemental security income (ssi); notarized letter from employer that states income earned weekly/monthly if family does not file taxes.
